NPT Gateway

Adults in Neath Port Talbot who need support to maintain or regain their independence can access the Community Resource Team and adult social care through a joint service with a common access point. This is called the NPT Gateway.

It is made up of a number of highly skilled contact officers, together with a core team of multi-disciplinary skilled professionals including a nurse, occupational therapist and a senior practitioner social worker.

They provide advice and signposting where necessary and make early assessments to identify people who need support from health or social services professionals.

Members of the public, GPs and other health and social services professionals can refer into the NPT Gateway. You can contact the NPT Gateway yourself or another person can contact them on your behalf.
